At a time when the crisis in health systems is increasingly visible, a silent phenomenon threatens their institutional solidity: the erosion of trust. This is not a matter of mere perception, but a progressive dismantling of the foundations that uphold the legitimacy, coherence, and authority of health-related decisions.<\/p>\n

Institutional trust is not an accessory; it is a strategic asset. It underpins adherence to clinical decisions, professional loyalty, user safety, and political-institutional investment. Its absence does not merely generate uncertainty, it directly undermines performance, organisational return, and the capacity to attract and retain talent.<\/p>\n

According to the OECD (2022), trust is one of the most relevant structural variables for the effectiveness of public policies. Gilson (2003) argues that in health, trust is not simply a result of good governance, but a foundational condition. Without it, decisions are challenged, processes stagnate, and the relationship with citizens deteriorates.<\/p>\n

As Fukuyama noted in 1995, trust is not a cultural ornament but a structuring element of complex institutions, granting them internal cohesion, decision-making stability, and the ability to coordinate across levels. His early analysis remains highly relevant: where trust is depleted, redundant control mechanisms emerge, resistance to change intensifies, and shared vision fractures.<\/p>\n

This perspective is deepened by authors such as Paul et al. (2020) and Greer et al. (2019), who describe trust as a prerequisite for organisational coherence, functional legitimacy, and sustainable performance in adaptive health systems. Paul et al. demonstrate that institutions with high levels of internal trust show greater efficiency, better adaptability to change, and superior clinical outcomes. The return is not merely symbolic or emotional: it translates into productivity, waste reduction, team stability, and sustainable indicators of institutional performance.<\/p>\n

It is not enough to attribute the erosion of trust to communication failures or mismanaged expectations. Although these factors may amplify negative perceptions, especially when discourse is erratic or unintelligible, the problem runs deeper: it is a collapse in the architecture of decision making. When authority channels are unclear, accountability is diluted, and decision-making processes are opaque or inconsistent, trust unravels. It is at this point that the true return on investment, begins to deteriorate. Not merely due to resource scarcity, but fundamentally due to the absence of strategically designed governance, capable of articulating decision-making, delegation, and management with systemic logic, functional coherence, and a sustainability-oriented vision. When decisions are fragmented, flows incoherent, and authority diluted across poorly connected levels, the result is not just inefficiency: it is structural waste. Opportunities are lost to optimise teams, strengthen units, correct investments, and consolidate reforms. In turn, the system\u2019s capacity to produce more, spend better, and sustain long-term performance is jeopardised.<\/p>\n

Without a robust internal reform one that revises institutional architecture, decision circuits, accountability mechanisms, and leadership criteria, it is not possible to generate real value. External micro-adjustments or sectoral measures will not resolve the essential issue: without structured trust and intelligible governance, there can be no institutional return.<\/p>\n

As Luhmann (1990) emphasised, trust functions as a mechanism for reducing uncertainty in complex systems, not merely as an emotional belief, but as a functional architecture that enables institutional action. Its absence, more than noise, compromises the very possibility of structured decision-making.<\/p>\n

In highly demanding contexts such as healthcare, trust cannot be delegated: it must be designed. There are systems where this construction is intentional, visible, and verifiable.<\/p>\n

In the United Kingdom, for example, the NHS has been consolidating readable decision-making structures and public accountability through functional organograms, accessible reports, and real-time dashboards, allowing both citizens and professionals to follow structural decisions with clarity.<\/p>\n

In Singapore, the SingHealth group combines institutional transparency with leadership stability and process coherence. Units integrate digital monitoring platforms, shared clinical protocols, and agile correction mechanisms, promoting a culture of trust supported by data and legitimised by results.<\/p>\n

In the Middle East, the King Faisal Specialist Hospital in Saudi Arabia shows how it is possible to combine technical rigour and formal authority within a robust governance model. Organisational clarity, well defined decision flows, and documented accountability create a high performance environment in which professionals know where they are, why and in service of what purpose.<\/p>\n

The functioning of these systems does not rely on individual voluntarism, but on institutional engineering in which trust is deliberately built, measured, and protected.<\/p>\n

Portugal currently faces a moment in which many of the problems in health are not merely resource based, but rooted in definition. What emerges is a growing opaqueness around who decides, how decisions are made, and on what basis. This lack of transparency generates not only external distrust but also internal disarticulation.<\/p>\n

This is not about blame, but about illuminating a blind spot: without clear decision-making structures, even correct decisions lose their impact. Trust is not eroded solely due to a lack of results, but due to the absence of structure. And this invisible erosion compromises everything else: motivation, efficiency, reputation, and internal fairness.<\/p>\n

Without reporting, there is no memory. Without memory, there is no accountability and where there is no accountability, error ceases to be corrected and begins to be inherited.<\/p>\n

Trust is not a vague feeling, it is a system. An invisible infrastructure, yet fully measurable in terms of performance, talent retention, commitment to institutional goals, and the very legitimacy of authority. Without it, what fragments is not just the relationship with the user, it is the very axis of governance.<\/p>\n

In healthcare, where margins for error are tight and time is critical, the absence of institutional trust has exponential costs: it compromises decisions, empties leadership, neutralises reforms, and installs a culture of hesitation.<\/p>\n

Rebuilding it requires far more than operational plans: it demands an architectural vision of governance, in which every decision is intelligible, every structure verifiable, and every leadership supported by functional legitimacy, not by political turnover.<\/p>\n
